Participant, Beneficiary & Payee Forms

 

Many of the applications and forms below are now "fillable". You can complete the application or form online, save it to your computer, print it, sign it and mail it to it the Fund.

 

 

The beneficiary designations you make on this form will be used only for a Pre-retirement Death Benefit.

Naming a Custodian for a Minor Beneficiary Form - Abiding by the Uniform Transfer to Minors Act (UTMA) when naming a Minor as Primary or Alternate Beneficiary.

             Apply for a regular pension.              Application for a pre-retirement death benefit.              Application for a post-retirement death benefit.
  • Notice for Foreign Resident Payee
    Requirements for Federal Income Tax Withholding for Pensioners and Beneficiaries receiving benefits and residing in a foreign country.
  • Covered Employment Correction Claim Form and Instructions
    Use this form to request a correction to the Covered Earnings and/or Pension Credit amount shown on your Annual Statement of Earnings. This form is to be completed if you believe upon review of your Annual Statement of Earnings that your covered employment has not been properly credited or reported to the Fund.
