Participant, Beneficiary & Payee Forms

Many of the applications and forms below are now "fillable".  You can complete the application or form online, save it to your computer, print it, sign it and mail it to it the Fund.

Pre-retirement Death Benefit Beneficiary Designation Form The beneficiary designations you make on this form will be used only for a Pre-retirement Death Benefit. Pension Application Apply for a regular pension. Disability Pension Application Apply for a disability pension. Attending Physician's Statement Determine disability eligibility. Death Benefit Application Application for a death benefit - applicable to post and pre-retirement death benefit. Pension Application for Participants of Age 70 1/2 Application for Participants who are eligible for a pension benefit and have reached age 70 1/2. Re-retirement Pension Application Application for Participants who are collecting a pension and are eligible for additional benefits earned after initial retirement but prior to Normal Retirement Age. Covered Employment Correction Claim Form and Instructions Use this form to request a correction to the Covered Earnings and/or Pension Credit amount shown on your Annual Statement of Earnings. This form is to be completed if you believe upon review of your Annual Statement of Earnings that your covered employment has not been properly credited or reported to the Fund.

Tax-Related Forms

Notice of Withholding for Non-Periodic Payments Form applicable to lump sum payments not eligible for a rollover. W-4P Federal Withholding Certificate Yearly withholding certificate for pension or annuity payments. W-4P State Withholding Certificate Yearly withholding certificate for pension or annuity payments applicable to the state of residence. W-9 Request for Taxpayer Identification & Certification The tax form that includes the Fund's taxpayer identification number (TIN) for entities that are required to file an information return with the IRS to report, for example, contributions paid to the Fund.

Other Forms

Application for Rollover Form applicable to lump sum payments eligible for a rollover. Rollover Distribution - Special Notice Explanation of tax consequences of a rollover distribution. Authorization to Release Information Authorize another person to receive information from the Fund Office on your behalf. Direct Deposit Establish or change direct deposit information for your monthly payments. Qualified Domestic Relations Order (QDRO) Forms Procedures to establish a QDRO. Model Pre-retirement QDRO A model QDRO for Participants who have not begun receiving pension benefits. Model Post-retirement QDRO A model QDRO for Participants who are receiving a pension benefit.

Local Forms

Local Participation Agreement Agreement for an AFM Local Union to participate in the Fund. Remittance Report Form  In order for the Fund to accept pension contributions, the contributions must be accompanied by a Remittance Report Form that includes specific engagement and musician details. Refer to Trust Document Section 9.8 for more information. Contribution Transmittal Agreement Agreement to follow contribution transmittal procedures. Electronic Cash Transmittal Form Form to be used in conjunction with the contribution transmittal procedures.

Employer Forms

Employer Participation Agreement Agreement for an Employer to participate in the Fund. Addendum to the Participation Agreement or Collective Bargaining Agreement for Limited Liability Companies Limited Liability Companies that wish to participate must complete the following form and submit it as an addendum to the CBA or Participation Agreement.
