Participant, Beneficiary & Payee Forms

  Many of the applications and forms below are now "fillable".  You can complete the application or form online, save it to your computer, print it, sign it and mail it to it the Fund.

 

The beneficiary designations you make on this form will be used only for a Pre-retirement Death Benefit.

  • Covered Employment Correction Claim Form and Instructions
    Use this form to request a correction to the Covered Earnings and/or Pension Credit amount shown on your Annual Statement of Earnings. This form is to be completed if you believe upon review of your Annual Statement of Earnings that your covered employment has not been properly credited or reported to the Fund.
