Re-retirement Pension Application Application for Participants who are collecting a pension and are eligible for additional benefits earned after initial retirement but prior to Normal Retirement Age.
Covered Employment Correction Claim Form and Instructions Use this form to request a correction to the Covered Earnings and/or Pension Credit amount shown on your Annual Statement of Earnings. This form is to be completed if you believe upon review of your Annual Statement of Earnings that your covered employment has not been properly credited or reported to the Fund.
W-9 Request for Taxpayer Identification & Certification The tax form that includes the Fund's taxpayer identification number (TIN) for entities that are required to file an information return with the IRS to report, for example, contributions paid to the Fund.
Remittance Report Form In order for the Fund to accept pension contributions, the contributions must be accompanied by a Remittance Report Form that includes specific engagement and musician details. Refer to Trust Document Section 9.8 for more information.
