Covered Employment Correction Claim Form and Instructions

If you believe any of the Covered Employment data shown on your Covered Earnings Reports is inaccurate, you must submit to the Fund Office a completed "Covered Employment Correction Claim Form" with all required documentation described on the form.  Please note:  There is a timing restriction for the filing of a correction claim form.  Details of this restriction are included in the instructions on the form.  You can download the form and instructions or use Contact Us to request that a form be mailed to you.

 

To access your Covered Earnings Reports online, you must be a registered user.  To register return to the Home Page and click on “Register Here” located in the lower left column.
